Most Recent Nursing Home Special Focus Facility List Released

Each year the Centers for Medicare & Medicaid release a list of Special Focus Facilities (SFFs). The purpose of the list is to identify nursing homes with serious deficiencies that have not been corrected. On average a nursing home has 6-7 deficiencies per survey. However, for nursing homes with more problems averaging twice the number of deficiencies, with more serious problems than the average facility, and a pattern of those problems repeating. Those nursing homes are added to the SFF list.

The facilities on the SFF list will receive surveys twice as frequently. If the deficiencies are not corrected within a sufficient amount of time, citations will be enforced with fines and possible termination from Medicare and Medicaid.   • Richmond Pines Healthcare And Rehabilitation Center (Hamlet, North Carolina)
  • Dover Center For Health & Rehabilitation (Dover, New Hampshire)
  • New Grove Manor (East Orange, New Jersey)
  • The Heights Of Summerlin, Llc (Las Vegas, Nevada)
  • Emerald South Nursing And Rehabilitation Center (Buffalo, New York)
  • Isabelle Ridgway Post Acute Care Campus Llc (Columbus, Ohio)
  • Pristine Senior Living & Post Acute Care Of Portsmouth (Portsmouth, Ohio)
  • Falling Spring Nursing And Rehabilitation Center (Chambersburg, Pennsylvania)
  • The Gardens At West Shore (Camp Hill, Pennsylvania)
  • Inspire New Boston (New Boston, Texas)
  • Pecan Valley Healthcare Residence (San Saba, Texas)
  • Envoy Of Westover Hills (Richmond, Virginia)
  • Pines Rehab & Health Ctr (Lyndonville, Vermont)
