"A" level citations involve a condition in which there is a substantial probability that death or serious mental or physical harm will result, or has resulted. When there is a condition or occurrence at the facility that proximately caused a resident’s death it is considered the worst violation at a rating of "AA".
The facilities were fined for their violations in amounts ranging from $25,000 to $50,000. The incidents involved included falls, medication errors, and pressure sores. For more, read the story.
